Step-by-Step: How to Get Your Immunisation History Statement

To enrol your child, you need one specific document: the Australian Immunisation Register (AIR) Immunisation History Statement. This is the only valid proof of immunisation. Here’s how you can get it:

Online via myGov

Log in to your myGov account, link your Medicare account, and download the statement instantly.

Through the App

Use the Express Plus Medicare mobile app to view and email the statement directly from your phone.

By Phone

Call the Australian Immunisation Register (AIR) at 1800 653 809. The statement will be mailed to you, but please allow up to two weeks for delivery.

In Person

Visit a Medicare or Centrelink office to request a printed copy on the spot.

Don't have a Medicare card?

You can still get a statement by asking your immunisation provider to print one for you, or by calling the AIR to have a copy mailed.

If you require assistance in a language other than English, you can contact the translating and interpreting Service at 131 450 , Monday to Friday, 8:30 am–4:45 pm.
